Biometric Screenings for the Local 400 Health & Welfare Fund for Active (Non-Medicare) Participants will be held in the following locations and times:

Please arrive 15 minutes prior to your appointment time to complete the health forms!

All Non-Medicare Active Participants and their Spouses will be required to participate in a Biometric Screening in Wellness Year 7 (July 1, 2019 through June 30, 2020). If you are retired, you and your spouse (if marrried) are NOT required to participate in a Biometric Screening, nor will either of you be assessed a penalty. Please arrive 15 minutes ahead of your scheduled screening to complete the health forms.

If you have participated in a Biometric Screening or submitted a form to your Primary Care Physician or Health Dynamics during the 7th Wellness Year of July 1, 2019 through June 30, 2020, you will not be eligible to attend another Biometric Screening during this period. If you are unsure if you and/or your spouse have health coverage at the time of an event, you should contact the Administrative Office prior to scheduling an appointment on the web calendar.

Simple steps to schedule your appointment are as follows:

Under “Client Login” the username is: local400 and the password is: pands400.
Once you are logged in, click on “local400″ >”health fund” > “calendar” arrow over to the month then on the date you want your appointment.
Click on the time slot that you would like to sign up for and fill out the form and submit. (If the time is unavailable, it will be grayed out.)
You will receive a confirmation of your appointment via e-mail.
If you need to cancel or change your appointment, contact the administrative office at 1-866-562-4004.

When setting up your appointment, you will be asked for your primary phone number. Preferably list your cell phone number, if possible. Your phone number, as well as your email address, will be shared with the Union Office. This information will assist in providing a higher level of communication for future Notices and text messages for the Biometric Screening Events. By providing this information to the Local 400 Health & Welfare Fund, you are allowing the Administrative Office the right to share this information with your Local Union Office.
